The effectiveness of botulinum toxin (BoTX-A) in 9 hemiparetic patients with therapy-resistant upper limb flexor spasticity was investigated. Graded doses of toxin (5-40 ng cumulative) were injected into 2 to 5 the most affected muscles. Documentation included assessment of Barthel-Index, Ashworth spasticity scale, Rivermead scale and patients and physiotherapists comments prior to and 3 month after injection. In three patients (8 ng toxin, 5 muscles each) spasticity could definitely be reduced with improved self-care abilities; selective movements were not achieved. Indications and limitations of BoTX-A therapy of severe spasticity are discussed.
