摘要
The advent of the Affordable Care Act has directed the national focus on the “triple aim,” reducing health care costs, and improving access and quality outcomes for populations.1 The goal is to create a culture of health that will enable all to live healthier lives regardless of socioeconomic status.2 This is at a time where there is an ever-increasing aging population, a need for chronic care management, and greater need for providing services in communities to meet their needs. So, where do nurses fit in as we embark on the journey of working together with communities to build a culture of health? © 2018 Mosby, Inc.
