The state of the art of mandibular surgery has rapidly progressed. Intermaxillary fixation has been replaced by rigid osteosynthesis performed through percutaneous stab wounds. The intraoral approach, however, has been hampered by the lack of proper instrumentation. We report on 27 patients in whom rigid fixation of mandibular osteotomies was accomplished transorally with perpendicularly placed bicortical screws using a specially designed retractor, an electric right-angle drill, and a manual and electric right-angle screwdriver.
