Osteochondral defects of the talus can be a source of persistent pain and disability after initial arthroscopic intervention. Secondary surgical options include osteochondral transplantation, which has shown consistently good results for this problem. Traditionally, exposure has been performed with a malleolar osteotomy. Anterior tibial osteotomy (plafondplasty) has also demonstrated adequate talar exposure for successful graft placement, with good clinical outcomes. The following article presents the anterior plafondplasty technique for exposure of osteochondral defects of the talus during osteochondral transplant surgery.
