Recent research on diverticular disease has concentrated on the structure of the colonic wall and the changes which occur with age. Collagen and elastin are particularly affected, leading to a decrease in tensile strength. The role of ispaghula and wheat bran in the treatment of symptomatic diverticular disease has been re-examined and a special role of the proximal colon in determining colonic flow and stool output is becoming evident.
