Analysis of hemodynamic parameters in patients with hypertension

Background. The choice of antihypertensive drugs, according to new recommendations of the European Society of Cardiology should be related to age, race, and comorbid conditions. Based on the non-invasive hemodynamic indices derived from impedance cardiography, there is the possibility of targeted pharmacotherapy, depending on the parameters obtained during examination. Objectives. Assessment of hemodynamic parameters in patients with controlled and uncontrolled hypertension. Material and methods. The study involved 199 patients treated for hypertension in the family doctor's office. Patients were divided into two groups: those with controlled blood pressure (< 140/90 mm Hg) (n = 104) and uncontrolled blood pressure (>= 140/90 mm Hg) (n = 95). The groups were homogeneous in age, duration of hypertension, body mass index. Hemodynamics examination was performed using an impedance cardiograph. During examination blood pressure, heart rate, cardiac index, systemic vascular resistance index and total fluid content were recorded. Results. The mean systolic and diastolic blood pressure in the controlled hypertension group, was 125.1 +/- 10.1 mm Hg and 77.7 +/- 8.0 mm Hg. In the group with uncontrolled blood pressure 152.7 +/- 18.2 mm Hg and 89.2 +/- 12.2 mm Hg. Fast heart rate, high cardiac output index, high peripheral resistance index and increased total fluid content in the group of uncontrolled blood pressure were measured significantly more likely. Conclusions. Hemodynamic indices significantly differentiate patients with controlled and uncontrolled hypertension.
