Postoperative delirium (POD) is a common complication of surgery associated with significant morbidity and mortality. Transfusion of packed red blood cells has been reproducibly associated with POD in many retrospective and prospective cohort studies. We review the biological evidence for a potential causal pathway between transfusion and delirium via oxidative stress and neuroinflammation, and summarize the clinical literature on delirium and transfusion to date. © 2014, Springer Science + Business Media New York.
