The diagnostic algorithm of dyspeptic symptoms is initially directed to exclude defined organic disorders which could explain the symptoms and we give rise to clearly directed causal therapy. If these disorders can be excluded by medical history, basic laboratory test and routine diagnostic a initial probatory therapy can be initiated. If alarm symptoms or atypical symptoms exist further examinations and tests are warranted. The therapeutic alternatives involve phytotherapy in all patients, acid suppression with H 2-blockers and PPI or in patients with proven Hp-positive gastritis a probatory eradication. These three therapeutic alternatives have been proven in randomized studies. Other possible alternatives are either in approval stages or failed to show consistent therapeutic effects. Several new drugs and alternatives have been suggested for the treatment of functional dyspepsia, which might offer new therapeutic options. © 2013 Springer-Verlag Berlin Heidelberg.
