Medical workstation for the rehabilitation of disabled people

It has been developed a workstation (Chronic Care Workstation-CCWS) for the rehabilitation and the chronic patients follow-up that facilitates the access to the medical professionals -in particular in the field of the rehabilitation- to an information of support that permit them to sustain a better attention and follow-up of their patients, The CCWS allows the managing of patient's data, the presentation of their clinic history, the elaboration of a patient's agenda (activity modules programmed in a concrete moment) and a future care plan that can imply the medical agents interaction and social services. Furthermore, it makes possible the conference of generalized rehabilitation protocols and the customization of these protocols in a concrete patient.
