摘要：
Transitional-age youth with chronic medical illness are a particularly vulnerable population that are at higher risk of mental health issues and psychosocial stress. It is important that both medical and mental health issues be addressed in a developmentally and culturally informed manner, as they transition from the long-term relationships they have built in pediatric settings to adult health care providers.
