Thyroid nodules: how to assess the risk of malignancy?

摘要
About 90 to 95% of thyroid nodules are benign and will required a basic follow-up. Ultrasound evaluation and cytological analysis for selected nodules, with formal reports made according to respective EU-TIRADS (European -Thyroid Imaging Reporting and Data System) classification and Bethesda system, can provide a quantification of malignancy risk and allow to adjust clinical and sonographic monitoring and to elaborate surgical treatment. In case of non-diagnostic (Bethesda 1) or indeterminate (Bethesda III and IV) cytologie one has to discuss reassessment and follow-up, and second line tools like elastography, scintiscan imaging and cytological molecular markers, if cost-effective.
