摘要
The aims of these guidelines are to improve the inpatient experience and safety for people with diabetes through effective self-management. The guidelines are aimed primarily at healthcare professionals working in hospitals, although some aspects are relevant to staff involved in pre-admission preparation. The guidelines suggest an approach to providing patient information, the circumstances in which self-management is appropriate, the development of care plans and the elements needed for effective self-management. This document is an abridged and modified version of Self-management of diabetes in hospital' adapted specifically for Diabetic Medicine.
