Distraction osteogenesis for the reconstruction of irradiated mandibule

摘要
We treated 5patients with a mandibular defect after tumor abrasion using distraction osteogenesis (DO) in an irradiated area. DO was applied for primary reconstruction in 1 site of 1 patient and secondary reconstruction in 7 sites of 4 patients. Satisfactory new bone formations were obtained at 5 sites in 4 patients. Partial callus formation was observed in 1 site. Only fibrous callus formation was observed in 1 site. From the results, we think that DO may be applicable even in an irradiated mandible. In addition to radiation, other factors such as the condition of soft tissue, vascularity of the bone, infection of the surgical site and the patient's general condition might influence the amount of callus formation. Further experiences will be necessary to determine the indication of DO procedure in an irradiated mandible.
