The health assurance coaching model for chronic renal disease management

摘要
Health assurance coaching is an approach to patients with CRD that emphasizes the common cause among caregivers and patients and is expressed in the coach-player relationship. Beginning with encouragement, building self-worth, giving information in stages, and the availability of the professional coaches, the primary responsibility for successful self-maintenance is progressively and ultimately vested in the players. Experience with this approach among patients in the Modification of Diet in Renal Disease Study showed their success in becoming competent and confident managers of their own medical and nutritional programs and healthy lifestyles. If the obstacles to this approach can be overcome and pre-end stage patients can be involved early, the desirable goals for patients with CRD renal disease in the 21st century may be within reach.
