摘要：
All over the world the Electronic Healthcare Record (EHR) evolves into a vital instrument in transmural healthcare, hospital extended homecare and telemedicine applications. Today, patients are expected to assume an active role in disease management and self-care, and to be able to use medical devices at home. An extension of the EHR is described that allows the patient to view personal medical information, to add personal comments and data gathered by medical devices and to share this information with caregivers. This extension is a Personal Electronic Healthcare Record (PEHR).
