Paget's disease of bone, an often undiagnosed metabolic bone disease, can lead to dramatic skeletal changes with enlargement and bowing of the affected bones. The article gives an overview on recent perspectives on surgical treatment options. Fracture fixation, total joint replacements, osteotomies of long bones and spinal decompression are the most relevant surgical procedures in Pagets disease. Along with preoperative planning of the intervention itself, a pharmacological anti-paget treatment with bisphosphonates is mandatory to reduce perk and postoperative complications.
