Anatomical basis of the medial adiposofascial flap of the leg

Despite recent advances in microsurgical techniques, coverage of lower leg defects by locorcgional flaps remains indicated in selected cases. We present the anatomical study of an adiposo-fascial flap that can be harvested on the anteromedial aspect of the leg and that can be mobilized to cover the area between the patella and the heel. This flap is based on the rich vascular network supplied by die saphenous artery and the posterior tibial artery perforators. This blood supply makes it possible to harvest this flap in various ways. The multiple harvesting techniques allowing to cover various defects will be discussed as well as the minimal donor site morbidity and maximal safety of this flap.
