Ileal-pouch anal anastomosis (IPAA) is the operation of choice for patients requiring proctocolectomy for chronic ulcerative colitis. The commonest long-term complication after IPAA continues to be the poorly understood and often misdiagnosed entity of pouchitis. The etiology, evaluation, and management of pouchitis are explored.
