BACKGROUND: Intramural pregnancy, accounting for < 1% of ectopic pregnancies, is a rare but potentially life-threatening condition due to the potential for uterine rupture. In most cases, the diagnosis of intramural pregnancy is not made until uterine rupture or surgery. CASE: A 29-year-old primigravid woman was referred for a suspicion of choriocarcinoma with persistent elevated serum beta-hCG. She had previously undergone a dilation and curettage procedure and discontinuous methotrexate therapy at the local hospital. The diagnosis of intramural pregnancy was confirmed, and the lesion was removed surgically. CONCLUSION: The preoperative diagnosis of intramural pregnancy is difficult, but current diagnostic tools can provide important clues. Increasing the awareness of this disease is of great importance.
