摘要：
Diabetes is a complex, multifactorial disease that permeates every aspect of a patient's life and often the lives of their friends and families. While managing a disease that heavily depends on lifestyle choices, a patient-centered care model is essential. Providers should use a variety of strategies focused on patient-specific goals and barriers while developing a detailed, individualized care plan.
