Clinical aspects of immediate implant placement and immediate loading

Benefits of immediate implants are an overall reduced treatment time and in combination with immediate loading the possibility to use fixed provisional restorations. The attainment of mandatory primary stability in ideal implant position and soft tissue management are especially challenging. Survival rates of immediate implants are in line with the outcome of delayed or late implant placement. Furthermore, immediate loading is as successful as other prosthetic loading protocols. The attention to risk factors as well as careful patient selection is extremely relevant to achieve good esthetic outcomes, particularly because immediate implants have a higher chance to establish recessions. The performance of alveolar ridge preservation has a positive effect on soft and hard tissue development and therefore is recommended. Besides the correct assessment of morphologic and patient individual risks, it is important to choose a suitable implant system. This article aims to present clinical aspects of immediate implants, its theoretical principles and its practical implementation on the basis of two illustrated case reports.
