Pharmacologic therapy continues to play an important role in the management of cardiac arrhythmias. Choosing an antiarrhythmic agent involves understanding the concepts related to arrhythmogenesis, risk stratification of patients, and antiarrhythmic mechanisms as well as the pharmacokinetics and pharmacodynamics of individual antiarrhythmic agents. This review focuses on some practical issues related to antiarrhythmic pharmacologic therapy with individual class I and class III antiarrhythmic agents currently in clinical use in the United States.
