A female triathlete with negative T-waves. Clinical case study

Negative T-waves often cause difficulties in differential diagnosis. A structural cardiac disease always has to be ruled out. Physiologically, negative T-waves may be caused by endurance training. We describe a young female triathlete with negative T-waves and exercise-induced ventricular tachycardia without evidence of structural cardiac disease in non-invasive and invasive diagnostics. After insufficient medical treatment, catheter ablation restored the full cardiovascular capacity. This case emphasizes that a pathologic cause of negative T-waves has to be kept in mind even if no definitive cause was detected by non-invasive and invasive diagnostic procedures. Control examinations during the follow-up are a good policy in patients without further clinical pathological findings.
