Vascular dementia or dementia of Alzheimer's type: The clinical differentiation.

摘要：
The clinical differentiation between degenerative dementia and vascular dementia proves inadequate in about one third of routine patients. Especially Binswanger's disease may mimic the clinical course of Alzheimer's disease. Cerebral computerized tomography, magnetic resonance tomography, single photon emission computerized tomography, and electroencephalography help in the differentiation in at least some cases. Hachinski's ischemic score has been validated by clinico-pathological studies and some items have proven useful for this differentiation: Step-wise deterioration, daily fluctuations, arterial hyper tension, focal neurological signs and symptoms, and extracerebral arteriosclerosis.
