Online discharge notes increase utilization of computerized medication lists

摘要
Recognizing that the moment of discharge from an inpatient to an ambulatory setting is a data-intensive and potentially high-risk step in the coordination of care, our research team conducted a study to evaluate the effectiveness of direct-entry discharge documentation in increasing the use and accuracy of the online medication list. Accurate medication data is a crucial element of post-discharge ambulatory care.(1,2,3) Any innovation that enhances the quality of medication information at time of discharge is likely to prevent post-discharge adverse outcomes.
