THE EFFECTIVENESS OF LOCAL CRYOTHERAPY IN THE TREATMENT OF PANCREATIC NECROSIS

The high morbidity and mortality make pancreatic necrosis (PN) a challenging issue of abdominal surgery. Local cryotherapy produces anti-inflammatory, cytostatic, anti-cytokinetic, anti -enzyme, antibacterial, hemostatic, analgesic, immunomodulating and regenerative effects. The bioplasticity and response of different parts of pancreas to LCT, the amount of blood perfusion, the coefficients of heat capacity and thermal conductivity, the parameters of the organ should be taken into account. 97 patients with infected pancreatic necrosis were subjected to contact LCT. The comparison group consisted of 116 patients. The groups were identical in age and severity of the disease. Titanium nickelide cryoapplicator with the tip temperature of -140-160 C was applied in 3-4 points for 20-30 seconds with the cycle repeated 2-3 times. A more prompt healing, 12% decrease in purulent -septic complications, 3.1% decrease of bleeding frequency and 12.2% lower postoperative mortality were reported in the experimental group. The results demonstrate a significant clinical effect of LCT in the surgical treatment of pancreonecrosis.
