An incidental finding of a tumor in the gastric wall by computer tomography shows a polycystic septate hypo-echoic mass in the submucosal layer in the endosonography. The endosonographic fine-needle puncture of this cystic tumor extracts a mucinous secretion without analysis of tumor-markers or pancreatic lipase. An endoscopic submucosal tunneling an resection of the tumor is not possible, because of adhered tissues of the submucosal layer an the muscular layer. The submucosal cystic tumor was therefore treated with endoscopic a laparoscopic wedge resection. Histology detects a heterotopic pancreatic tissue in the gastric wall with enlarged cystic pancreatic ducts and low grade neoplasma and expression of p53. Intraductal papillary mucinous neoplasm (IPMN) in heterotopic pancreatic tissue in gastric wall is a very rare finding and published only few times in the literature.
