COMPARE THE EFFECTIVENESS OF USING LYOPHILIZED COLLAGEN TYPE I MATRIX WITH CONVENTIONAL TREATMENT IN OPEN WOUND HEALING AFTER SURGERY FOR PILONIDAL SINUS

Objective: There is no consensus on the closure technique of the midline defect after excision of pilonidal sinus. Secondary healing after excision and marsupialization is a long process and causes work loss and increases treatment cost. Therefore, researchers have directed their efforts to improve wound care products that accelerate wound healing For this reason, we aimed to compare the effectiveness of lyophilized collagen type 1 matrix with conventional treatment on secondary healing. Material and Method: A total of 55 (female/male: 11/44) patients were included in the study who were treated by secondary healing following excision and marsupialization. On group I, the wound was cleaned daily with saline and povidone iodine and closed with gauze, on group II (n=28), the wound care was made 3 times a week using sterile lyophilized collagen type I matrix. Results: There was no statistically significant difference between the groups regarding to age, BMI and initial wound volume. Total wound healing time is longer in group I than in group II, 7:07 (3-12) and 6.3 (4-12) weeks, respectively. Treatment cost was 420 $ in group I, while 585 $ in group II (p=0.042). Recurrence was 4.3% in group I and there was no recurrence in group II. Conclusion: Lyophilized type I collagen matrix use for secondary healing of pilonidal sinus significantly accelerates wound healing and provides an important advantage with low recurrence rates.
