The quality assurance programme in the North-Rhine area of Germany documents about 11 000 laparoscopic gall bladders per year and per ten million inhabitants. In 1994 51 lesions of the common bile duct were documented. A structured anonymous questionnaire was sent to all the hospitals that reported a common bile duct injury. The results of this questionnaire were analyzed according to a failure analysis strategy. The aim was to develop a strategy to avoid common bile duct injuries in the future.
