Predictors of sexual behaviour among men and women in New York City area prisons

The USA currently has the highest incarceration rate in the world. Individuals at greatest risk for HIV also tend to be among those at an elevated risk for incarceration. The HIV epidemic is driven by injection drug use and sexual behaviour, and estimates of prison-based sexual behaviour vary widely. This study examined sexual behaviour among 1369 incarcerated men and women in the New York City area. Approximately 13% of the sample (n = 176) reported being sexually active while incarcerated over the previous 6months. Multiple logistic regression models were used to examine the predictors of prison sexual behaviour. The following variables were associated with increased odds of sexual activity in the previous six months: female gender (OR = 3.28; 95%CI = 1.95,5.51), Black race (OR = 2.26; 95%CI = 1.47,3.46), history of drug use (OR = 1.85; 95%CI = 1.04,3.30), bisexual (OR = 3.19; 95%CI = 1.90,5.38) or homosexual identity (OR = 8.50; 95%CI = 3.92,18.43) and conjugal visitation programme participation (OR = 66.26; 95%CI = 31.42,139.74). Educational programmes regarding sexually transmitted infections and appropriate harm reduction measures are warranted for this population. The expansion of conjugal visitation programmes may also be useful in helping prisoners sustain their relationships with partners from their community and reduce the risk of infectious disease transmission in this environment. Les individus les plus exposes au risque lie au VIH sont aussi fortement exposes a celui d'une incarceration. L'epidemie de VIH est alimentee par la consommation de drogues injectables et les comportements sexuels, et les estimations sur les comportements sexuels en prison varient considerablement. Cette etude a examine les comportements sexuels parmi 1369 hommes et femmes incarceres dans la region de New-York. Environ 13 % de ces personnes (n = 176) ont rapporte qu'elles etaient sexuellement actives alors qu'elles etaient incarcerees au cours des 6 mois precedents. L'analyse par regression logistique multiple a ete employee pour examiner les facteurs predictifs des comportements sexuels en prison. Les variables suivantes ont ete associees a de plus grandes chances d'avoir une activite sexuelle en prison au cours des six derniers mois : genre feminin (OR = 3.28 ; IC 95 % = 1.95, 5.51), race noire (OR = 2.26 ; IC 95 % = 1.47, 3.46), histoire d'usage de drogues (OR = 1.85 ; IC 95 % = 1.04, 3.30), identite bisexuelle (OR = 3.19 ; IC 95 % = 1.90, 5.38) ou homosexuelle (OR = 8.50 ; IC 95 % = 3.92, 18.43) et participation a un programme de visites conjugales en prison (OR = 66.26 ; IC 95 % = 31.42, 139.74). Des programmes d'education sur les infections sexuellement transmissibles et des mesures appropriees de reduction des risques s'imposent dans cette population. L'expansion des programmes de visites conjugales devrait aussi aider les prisonniers a preserver leur relation avec des partenaires issus de leur communaute et a reduire le risque de transmission de maladies infectieuses dans cet environnement. ResumenEstados Unidos presenta en la actualidad la tasa de encarcelamiento mas alta del mundo. Asimismo las personas con un elevado riesgo de contraer el virus del sida son las que corren mas riesgo de encarcelamiento. La epidemia del sida esta condicionada por el consumo de drogas por via intravenosa y la conducta sexual; y las estimaciones por la conducta sexual en las prisiones varian de manera significativa. En este estudio analizamos el comportamiento sexual de 1369 hombres y mujeres que estan en centros penitenciarios en el area metropolitana de Nueva York. Aproximadamente un 13 por ciento de la muestra (n = 176) informo haber sido sexualmente activo en los ultimos 6 meses durante el periodo de encarcelamiento. Utilizamos modelos de regresion logistica multiples para examinar los indicadores del comportamiento sexual en las prisiones. Se asociaron las siguientes variables con una mayor probabilidad de actividad sexual en los seis meses previos: genero femenino (RG = 3,28; 95% IC = 1,95, 5,51), raza negra (RG = 2,26; 95% IC =1,47, 3,46), antecedentes de consumo de drogas (RG = 1,85; 95% IC = 1,04, 3,30), identidad bisexual (RG = 3,19; 95% IC = 1,90, 5,38) u homosexual (RG = 8,50; 95% IC = 3,92,18,43), y participacion en programas de visitas conyugales (RG = 66,26; 95% IC = 31,42, 139,74). Se recomiendan programas educativos sobre infecciones de transmision sexual y medidas apropiadas para reducir los riesgos en esta poblacion. Tambien seria util ampliar los programas de visitas conyugales para que los presos conservaran las relaciones con parejas de su comunidad, reduciendo asi el riesgo de transmision de enfermedades infecciosas en este entorno.
