THE ANATOMIC DOUBLE BUNDLE RECONSTRUCTION OF ANTERIOR CRUCIATE LIGAMENT Aim. The single-bundle ACL reconstruction with hamstrings is the most common technique to restore knee stability and the good results obtained demonstrate its validity. Clinical review, however, show that the success rates of this technique are not always satisfactory and in vivo studies showed that the rotation can remains excessive during the most demanding sport requests. This report presents the technique studied and developed by Volpi to perform an anatomical double-bundle ACL reconstruction and a comparison of results obtained with this technique and other single-bundle reconstructions. Methods. We evaluated 52 patients, 29 underwent single-bundle reconstruction (SB) and 23 with double-bundle reconstruction (DB). All patients have filled in a self-evaluation form and have been subjected to clinical examination. Results. The DB group have gotten higher score in subjective evaluation, Lachman test, anterior drawer and pivot shift test. The evaluation of varus-valgus stress is been more favorable in the SB group. All other tests have provided results comparable between the two groups. Conclusion. The anatomic double bundle ACL reconstruction is more complicated than single bundle reconstruction but, through the use of the guides, standardization and reproducibility are possible. This is a relatively new surgery technique so another clinical studies are needed to determine its clinical utility, however, we believe that the anatomic double-bundle reconstruction can provide patients with ACL injury a valid surgical option.
