摘要
Due to ageing of the population, an increase in the incidence and prevalence of diabetes mellitus can be expected in Germany in the coming years. This disease causes a high disease burden and costs for the social insurance systems. Therefore, attempts were made many years ago to organise the care of people with diabetes more effectively through guideline programmes and the introduction of disease management programmes. Only in the recent past has the intensive involvement of patients in their own therapy process become more of an issue. The project Personalised Self-Management Support Programme (P-SUP), funded by the Innovation Fund of the German Joint Federal Committee, aims to specifically promote the self-management of chronically ill people.
