摘要
Die molekulare Diagnose ist wichtig, um eine Allergen-Immuntherapie bei Kindern mit allergischer Rhinokonjunktivitis und/oder Asthma pr & auml;zise zu verschreiben. Sie erm & ouml;glicht die Unterscheidung zwischen Patienten, die auf Extrakte im Hauttest oder im IgE-Test reagieren, und Patienten mit oder ohne Sensibilisierung gegen spezifische oder ,,echte" allergene Molek & uuml;le. Diese & Uuml;bersichtsarbeit pr & auml;sentiert die Grundlagenkonzepte einer molekularen Allergiediagnostik, unter anderem Allergenfamilie, Panallergene, kreuzreaktive und ,,genuine" Molek & uuml;le, B-Zell-Epitope, Allergen-Microarray, Inducer- und Elicitor-Molek & uuml;le, Initiatormolek & uuml;le, ,,molecular spreading", ,,major-" und ,,minor"-allergene Molek & uuml;le. Zuk & uuml;nftige Entwicklungen in der In-vitro-Molek & uuml;ldiagnostik f & uuml;r IgE-vermittelte Allergien, unter anderem die Analyse des Nasensekrets sowie Allergiescreening, basieren auf einer Kombination aus Initiatormolek & uuml;len und klinischen Entscheidungsunterst & uuml;tzungssystemen. Viele weitere Entwicklungen werfen ebenfalls interessante Perspektiven auf. Molecular Diagnosis is important in order to accurately prescribe Allergen Immunotherapy for Children with allergic Rhinoconjunctivitis and/or Asthma. It allows Differentiation between Patients who react to Extracts in the Skin Test or in the IgE Test and Patients with or without Sensitization to specific or "real" allergenic Molecules. This Review presents the Basic Concepts of molecular Allergy Diagnostics, including Allergen Family, Panallergens, Cross-reactive and genuine Molecules, B-Cell Epitopes, Allergen Microarray, Inducer and Elicitor Molecules, Initiator Molecules, molecular spreading, major and minor allergen Molecules. Future Developments in In vitro Molecular Diagnostics for IgE-mediated Allergies, including Nasal Secretion Analysis and Allergy Screening, will be based on a Combination of Initiator Molecules and clinical Decision Support Systems. Many other Developments also raise Interesting Prospects.
