Augmentation and implant-prosthetic reconstruction after extensive periodontal attachment loss

摘要
This case report presents fixed implant prosthetic restoration in a 72-year-old female patient with advanced bone loss in the maxilla and mandible. Causes were marginal periodontitis, a widely caudally extended maxillary sinus floor on both sides, and alveolar ridge atrophy due to long-term edentulism. After extensive consultation, the patient opted for an implant-supported dental prosthesis, which was realized by extensive augmentation with bilateral external sinus lifts in combination with the tooth shell technique, using a mixture of the dentin of the removed teeth (tooth shell technique), autologous bone from the facial maxillary sinus wall and the retromolar region of the mandible, and a bovine bone substitute. The patient's wish for maximum tooth retention, which is also definitely advantageous from a physiological point of view, and the avoidance of a removable denture could be met in this way.
