Subchondral insufficiency fractures of the knee (SIFK) are a relatively common cause of knee pain, particularly in middle-aged and older adults. The SIFK is a type of stress fracture that occurs when excessive and repetitive or supraphysiologic loads are applied to subchondral bone [1]. Historically, this type of fracture was termed spontaneous osteonecrosis of the knee (SONK) until advances in MRI identified underlying fractures as well as meniscal deficiency as likely attributable etiologies. Consequently, SIFK has replaced SONK as the more appropriate term to refer to this category of conditions, with SONK now viewed as an advanced SIFK lesion. With greater availability of MRI, SIFK has been more frequently recognized and not as commonly mistaken for knee osteoarthritis as it had been in the past, with important implications for treatment and management of this condition. Subchondrale Insuffizienzfrakturen des Kniegelenks (SIFK) sind eine relativ h & auml;ufige Ursache f & uuml;r Knieschmerzen, insbesondere bei Erwachsenen mittleren Alters und & auml;lteren Menschen. Die SIFK ist eine Art der Stressfraktur, die durch die Einwirkung & uuml;berm & auml;ss iger und wiederholter oder supraphysiologischer Belastungen auf den subchondralen Knochen auftritt. In der Vergangenheit bezeichnete man diese Frakturen auch als spontane Osteonekrose des Knies (SONK), bis durch Fortschritte in der Magnetresonanztomographie (MRT) die zugrunde liegenden Frakturen sowie Meniskusdefekte als wahrscheinliche Ursachen identifiziert wurden. Folglich hat sich der Begriff SIFK als zutreffendere Bezeichnung f & uuml;r diese Erkrankung gegen & uuml;ber der SONK durchgesetzt, wobei SONK heute als eine fortgeschrittene SIFK-L & auml;sion angesehen wird. Durch die gr & ouml;ss ere Verf & uuml;gbarkeit der MRT wird die SIFK immer h & auml;ufiger erkannt, und es kommt seltener zur Verwechslung mit einer Kniearthrose, als dies in der Vergangenheit der Fall war, was bedeutende Auswirkungen auf die Behandlung und das Management dieser Erkrankung hat.
