Autologous augmentation using a compressed stump of the long head of the biceps tendon (LHBT) offers a promising approach for treating large rotator cuff tears (LRCT). This case report highlights the successful application of this technique. A 77-year-old active female presented with right shoulder pain and restricted mobility. Initial MRI revealed multiple injuries, including a complete supraspinatus tear and a partial infraspinatus rupture. Following arthroscopic rotator cuff repair, subacromial decompression, and suprapectoral tenodesis of the LHBT, the remaining stump was used as an augmentation patch for the supraspinatus tendon. Six months postoperatively, the patient demonstrated full, pain-free mobility and successful patch integration. This case underscores the potential of the LHBT as an autologous graft to enhance healing and stability in LRCT. As a promising technique, LHBT augmentation could provide an effective solution for managing large rotator cuff tears, particularly in patients at high risk of retear. Die autologe Augmentation mit einem komprimierten Stumpf der langen Bizepssehne (LBS) ist eine vielversprechende Technik zur Behandlung gro ss er Rotatorenmanschettenrupturen. Dieser Fallbericht beschreibt die erfolgreiche Anwendung dieser Methode. Eine 77-j & auml;hrige, aktive Patientin stellte sich mit Schmerzen und eingeschr & auml;nkter Beweglichkeit in der rechten Schulter vor. Das initiale MRT zeigte eine komplette Supraspinatusruptur, eine partielle Infraspinatusruptur sowie weitere Begleitverletzungen. Nach einer arthroskopischen Rotatorenmanschettenrekonstruktion, subakromialen Dekompression und suprapektoralen Tenodese der LBS wurde der verbleibende Stumpf als Augmentationspatch f & uuml;r die Supraspinatussehne verwendet. Sechs Monate nach der Operation zeigte die Patientin eine vollst & auml;ndige, schmerzfreie Beweglichkeit sowie eine erfolgreiche Integration des Patches. Dieser Fall unterstreicht das Potenzial der LBS-Augmentation als vielversprechende Technik zur Verbesserung der Heilung und Stabilit & auml;t bei gro ss en Rotatorenmanschettenrupturen. Die Methode k & ouml;nnte eine effektive L & ouml;sung f & uuml;r die Behandlung gro ss er Rotatorenmanschettenrupturen darstellen, insbesondere bei Patienten mit hohem Risiko f & uuml;r eine Re-Ruptur.
