It won't happen to me

An accident in a concrete formwork crew during the building of a machine base in a industrial plant was described. The crew used gang forms to encase the mass concrete and tons of reinforcing steel. High accuracy was needed because huge base anchor bolts to secure the machine had to line up precisely. Damaged corner chamfer strip was noticed while the crew guided the huge form into place. The operator was asked to hoist the heavy form just high enough for a carpenter to get in to make repairs. In an attempt to do repair works, the heavy form fell on the repair carpenter and the person was injured seriously.
