In recent decades, technological advancements have facilitated the discovery of several novel antibody-mediated diseases, each presenting with a diverse and heterogeneous array of clinical manifestations. Conventional neurologic disability assessment tools, such as the modified Ranking Scale (mRS),(1) although useful, often fall short in encapsulating the complex symptomatology of these disorders, focusing predominantly on ambulation and activities of daily living. Similarly, the Expanded Disability Status Scale (EDSS),(2) which has been used in multiple sclerosis and related disorders, despite offering a more comprehensive neurologic assessment, misses several aspects of these conditions. Development of a composite score for autoimmune encephalitis (CASE) score has been a promising step in development of disease-specific outcome measures.(3) However, many items included in the CASE score may predominantly apply to NMDA receptor immunoglobulin G-mediated autoimmune encephalitis, not to other syndromes such as anti-IgLON5 disease.
