Off-label drug use (OLDU) refers to the use of FDA-approved drugs for unapproved uses. OLDU is common in many patient populations, especially in children and the elderly. While the FDA approves drugs for specific uses, it does not regulate how drugs are prescribed. However, the FDA limits how drugs may be promoted for off-label uses and has issued large fines to manufacturers who have violated its guidelines. The practice of OLDU and the role of the FDA in regulating promotion will be discussed.
