Brachymetatarsia (congenital shortening of the metatarsal) results in cosmetic deformities and can result in transfer metatarsalgia. The surgical management of this condition requires lengthening. The goal is to restore the parabolic arch of the foot and the length of the metatarsal to normalize the load transfer across the metatarsal heads. This will improve the cosmesis as well. Distraction osteogenesis via external fixation has been used successfully in the gradual correction of brachymetatarsia. The aim of this article is to review the indications and surgical technique of distraction osteogenesis for brachymetatarsia of the first and fourth metatarsals.
