Transition from a psychological point of view

Health care transition comprises the planned and coordinated transfer of responsibilities to the young adult person and the new adult medicine health care providers. To organize this transition is mostly, but not exclusively, the task of pediatricians. Due to the glaring lack of evidence regarding particular elements of the respective transition processes it is so far difficult to differentiate between nice or needed. However, there is agreement that all communication and interventions have to be transparent and reliable.
