摘要：

A trust finance director argues that doctors are best placed to win the debate on better health care for all, but first must understand the realities of being part of central government and the financial consequences of this. Change will best be achieved by facing up to the financial issues and moving the system from within; clinical audit could prove to be the most powerful weapon.
