Narcolepsy with cataplexy, narcolepsy without cataplexy, and idiopathic hypersomnia are 3 major primary hypersomnias that are currently diagnosed mostly by sleep phenotypes. Diagnostic tests are needed to understand the mechanisms of disease and provide better treatments. Recent progress for understanding the pathophysiology of excessive daytime sleepiness (EDS) comes particularly from the discovery of narcolepsy genes (ie, hypocretin receptor and peptide genes) in animals in 1999 and the subsequent discovery of hypocretin ligand deficiency in idiopathic cases of human narcolepsy-cataplexy. Changes in other neurotransmitter systems may also mediate sleep abnormalities in hypocretin-deficient narcolepsy and other hypersomnias.
