One-stage bony reconstruction of both the maxilla and mandible with the same vascularized bone transfer has not been reported previously. In this case, subtotal maxillary reconstruction and hemimandibular reconstruction were performed simultaneously with one flap, the free fibular osteocutaneous flap. A second flap was required for reconstruction of the nasopharynx, and split-thickness rib grafts were used to replace the periorbital bone. © 1993 American Society of Plastic Surgeons.
