Although hemodilution in ischemic stroke - being the most frequent neurological cause of hospitalisation - is in use for more than 20 years, it is still a matter of debate. Extensive studies during the past 7 years using iso- and hypervolemic hemodilution with low molecular dextranes or hydroxyethyl starch were unable to unequivocally establish a beneficial effect of this regimen. Therefore the possible side effects - with dextranes more than with hydroxyethyl starch - consisting of anaphylactic reactions, volume overload of the cardiovascular system, cerebral hemorrhage and acute renal failure have to be considered all the more critically.
