BACKGROUND: Minimal invasive coronary surgery as compared with classical coronary artery bypass (CABG) avoids sternotomy. The least invasive procedure is totally endoscopic CABG (TECAB) METHODS: For this review we discussed the current literature and included our own experience at Innsbruck Medical University, Austria and University of Maryland, USA. RESULTS: Minimal invasive and endoscopic CABGs are complex and sometimes time-consuming operations which currently restrict them to dedicated centers. However, increased postoperative quality of life and earlier return to physical activities are benefits for the patients. CONCLUSIONS: Minimal invasive and endoscopic techniques can be offered in coronary bypass surgery with good early and long-term results.
