Cost analysis of diagnostic testing for urolithiasis [Kostenanalyse van de diagnostiek bij verdenking op urolithiasis]

Purpose: To analyze the costs of the current standard diagnostics in the detection of urolithiasis. Methods: All 390 DBCs in renal colics, opened from May 2010 until May 2011, were analyzed. 217 patients met the inclusion criteria (anamnestic colicky pain, not pregnant, data available in this hospital). Imaging results were noted. The actual costs made for imaging were compared to the costs that would have been made when all patients would only have undergone a CT scan. Results: In total 207 ultrasounds, 185 KUBs and 112 CT scans were performed. Costs for one KUB, ultrasound of the kidneys and CT of the abdomen were resp. € 41.34, € 102.77 and € 168.33. Actually made costs were € 47,774, calculated costs for 217 CTs were € 36,527. Conclusions: From a cost perspective, it can be considered to perform a non-contrast enhanced CT in all patients suspected for urolithiasis. This must be weighed against the risk of higher radiation exposure.
