Anatomical variations in the location of gall bladder are rare. Rarely, they may present with intra-operative surprise. We present a case of an ectopic located para-duodenal inflamed gall bladder with cholelithiasis that could not be pre-operatively localized as ectopic and turned out to be a surgical challenge.
