The first lumbar vertebra of drug (heroin) addicts was analyzed by computer-tomography and histomorphometry. The bone marrow density, determined by computer-tomography, was found to be lo or very low in 50% of the cases, but did not differ significantly from a group of control cases. The histomorphometric analysis of the structure of the vertebral spongiosa also revealed no signifiacnt differences between the drug users and the controls. However, the relationships between the bone marrow density and the morphology of the spongiosa differed in both groups. In the controls, subjects with low bone marrow density values also had relatively narrow trabeculae in the spongiosa and subjects with high values had relatively wide trabeculae. Among the drug users, these values varied independently. This may indicate an alteration of the bone metabolism among drug users.
