In patients with acute myocardial infarction, left ventricular free-wall rupture (LVFWR) is a well-recognized complication associated with high mortality. Accurate diagnosis of LVFWR allows successful surgical treatment and may improve survival rates. We report on two patients initially evaluated with a presumed diagnosis of thoracic aortic dissection. In both cases CT revealed the presence of LVFWR.
